Men's Tennis Try-outs Set for Aug. 23

OMAHA, Neb. - The Omaha men's tennis team will hold open tryouts for all full-time UNO students starting at 1:30 p.m. on Tuesday, Aug. 23 at the Koch Tennis Center. All players should bring their own equipment.
In order to participate in the tryout each player MUST submit the following five forms by Friday, Aug. 19:
1. Proof of Physical within last 6 months. (Click here for UNO Tryout Physical Form if needed)
2. Proof of Medical/Health Insurance. (Copy of front and back of insurance card)
3. Proof of Sickle Cell Testing & Results
4. Release for Athletic Participation Form
5. Tryout Clearance Form (Turn in to Compliance Office with your spring semester schedule to prove full-time status at UNO).
If you are under 18 forms must be co-signed by a parent or guardian.
The first four forms (proof of physical, proof of insurance, proof of sickle cell testing & results and release for athletic participation) must all be submitted to the UNO Athletic Training Room in the lower level of the Sapp Fieldhouse prior to 5 p.m. on Friday, Aug. 19.
The tryout clearance form with fall semester schedule must be turned into the UNO Compliance Office, located on the upper floor of the Sapp Fieldhouse.
Players are asked to contact head coach Tyson Thomas (tysonthomas@omavs.com) to let the Omaha coach know you are planning on attending tryouts and to confirm that you are not too old to try out.
